2011-2012
Ford Elementary Science Olympiad Application

Congratulations! You have taken the first step to joining the Ford Science Olympiad Team. Science Olympiad is a national organization dedicated to promoting science, and the coaches at Ford are interested in students who demonstrate a great interest in science, are self-motivated, responsible, work well with others and will make a commitment to attend the meetings and to compete with the team.

Our team will consist of eighteen motivated and highly qualified students. The National Science Olympiad Organization sets this team size. When we go to the State Competition in May, we can only have a team of eighteen Olympians compete. On the team of eighteen members, only eight can be from the fifth grade. The remaining ten will be from  third or fourth grade. We will also select two alternates who will practice with our team.

Your next step is to review the schedule of practices and events for conflicts with your other commitments, and then fully complete the 3-part application attached and submit the application by the due date. Please return only attached Part I, II and III (pages 3, 4 and 5) by September 16th to Mrs. Cauffiel. The schedule is listed on the next page. In this application you will need to get permission and an endorsement from your teacher, answer several questions and plan an experiment for a science demonstration after school.  You will be scored on your application and experiment. We, the coaches, will be looking for presentation skills and an understanding of the science concepts in the experiment when we observe your experiment.  The experiment does not need to be elaborate or costly.  The highest scoring students will be on the Ford team. Please remember in completing the application that spelling, grammar, punctuation, completeness and neatness are important communication tools to scientists.

The application may be word processed or handwritten. You may write on the back of pages or add a page if you need, but you need to keep the first two pages for your records.

Thank you for applying to the Ford Science Olympiad Team,
Your Coaches,

Mrs. Cauffiel, Mrs. Turner, Mrs. Holmes, Mr. Plunkert

Parental Permission and Support Statement

As the parent of the above student I think my child will make a good Science Olympian, and I give my permission for him or her to participate on the Ford Science Olympiad Team. I will do all I can to support the team with assistance and snacks and by having my child at all practices. I also understand that a parent needs to accompany the student on the day of the Cobb County Science Olympiad, March 3rd, and at the State Science Olympiad which will be on May 12th*.

If we had 6 parents at each practice to help students study and practice it would be great.
I will volunteer to help coach at scheduled practices.

I will help by sending in snacks. Please tell me when.

Science Olympiad Team Member Understanding Statement

As a Science Olympiad team applicant, I understand that my grades, my conduct and attendance both in school and at practice are important. I understand if my grades fall below a “C” for the 9-weeks in any subject, or if I am sent to the Principal for misconduct, or I miss more than two Science Olympiad practices without a written excuse, I may be excused from the team. I also understand that my attending the March 3rd  and May 12th* events are critical to my team, and I will not let other events cause me to miss my obligations to my team.
